Survey settings (or Form Settings) control the overall behavior, appearance, navigation, and functionality of a GOapply Phase. These settings allow you to determine how pages advance, how questions are displayed, validation behavior, and whether the form is read-only or interactive.
Use this guide to configure Survey Settings within Simple Form Builder in GOapply. Each section below describes available options and default behaviors.

1. Access Survey Settings
From a Phase record in akoyaGO, launch the Application Form Editor:
From within Simple Form Builder, click the Survey Settings icon in the top navigation bar to configure form properties:
2. General
- Make the survey read-only (toggle). By default, this will be toggled off. If you want all questions within this Phase to be read-only, you can turn this setting on. Remember, there is also a read-only option at a question level.
- Select a Survey language: Default is English (currently the only language supported by akoyaGO).

3. Logo in the Survey Header
- Survey logo: Paste an image link (no size limits) or click the folder icon to upload a file from your computer.
- File upload size limit: Up to 64 KB when using the folder (upload) option.
- Public image links: If using a link, ensure it can be pasted into any browser and the image loads (the link must be public).
- Logo width: Set width in CSS units (px, %, in, pt, etc.). Default = auto.
- Logo height: Set height in CSS units. Default = 40px.

Important: Use public image URLs for logos, or upload files under 64 KB via the folder icon.
4. Navigation
- Survey layout (option set):
- Show table of contents (TOC): Default = unchecked. A table of contents (TOC) shows an outline of form contents and helps users to quickly navigate through longer forms.
5. Question Settings
Question order: Keep original or randomize (visible in Preview tab and GOapply UI). Default = Original.
- Question title alignment: Applies at form level; can be overridden at panel, page, or question.

- Required symbol(s): Symbol(s) indicating an answer is required.
- Error message alignment:
- Focus first question on a new page: Select if you want the first input field on each page ready for text entry.
- Auto expand text areas: Auto-grows Long Text and comments.
- Allow users to resize text areas: Applies to Long Text and comments.
6. Pages
- Pages: Displays each page’s Page name and Page title; supports deleting entire pages.
- Show page titles: Default = checked.
- Show page numbers: Default = unchecked.
- Enable grid layout: Structures elements using columns. Default = unchecked.
7. Data
- Clear hidden question values: Choose when to clear values hidden by conditional logic:

8. Validation
- Set focus on the first invalid answer: Default = checked.
- Run validation:
9. “Thank You” Page
- Show the “Thank You” page: Default = checked.
- "Thank You" page markup: Define the message the user will see on the "Thank You" page
- Dynamic "Thank You" page markup: Allows for a dynamic Thank You page based on conditional logic. For example, if the applicant is ineligible but still able to submit - or a different message to appear based on Requested Amount.
