Make Middle Name Editable on the Contact Form (Replace Composite “Full Name” with Individual Fields)
Prerequisites
- You have access to customize the Contact main form in your akoyaGO Dataverse environment (Model-driven app form designer).
- You can open the Contact table and edit forms within a Solution (recommended) or the default solution.

Brief Overview of the Microsoft Feature (Composite vs. Individual Name Fields)
The Contact form often shows a single composite “Full Name” control. This control groups First, Middle, and Last Name and can make Middle Name appear read-only or hidden. To edit Middle Name directly, remove the composite Full Name from the form and add the individual First Name, Middle Name, and Last Name columns to the form using the model-driven form designer.
Important: If your organization relies on the system-generated “Full Name” value (e.g., in list views or mail merges), changing the form to show individual fields will not change how “Full Name” is calculated. That format is governed by System Settings (Full Name format) and applies to new records only.

1. Donor stewardship: Ensure legal names (including Middle Name) are collected and editable for pledge letters and tax receipts.
2. Scholarship records: Differentiate applicants who share First/Last names by capturing and editing Middle Name on intake forms.
3. Board/committee rosters: Maintain consistent naming standards for rosters and meeting minutes when Middle Name or initial is required.

Practical steps (summary):
• Open a Solution → Contact table → Forms → Edit the main form.
• Remove the composite Full Name control from the form canvas.
• Add individual columns: First Name, Middle Name, Last Name to the desired section.
• Save & Publish. Verify Middle Name is now visible and editable.
• (Optional) Review your System Settings → Full Name format if you also need Middle Name to appear in the calculated “Full Name” shown in views and headers. Note: System Settings changes affect new records going forward.
